Summary:
Reporting directly to the Manager of Care Coordinators, the Care Coordinator (CC) provides direct client care including safely transporting residents to events and appointments, aids other departments regarding client care, ensures clients awareness of individual and group sessions, and actively communicates with relative departments regarding client status and behaviors.
Duties and Responsibilities:
- Responsible for the safe transportation of all residents to activities, appointments, or other offsite events.
- Provides shift reports to the clinical and leadership team.
- Aids the admissions team in new resident intake in accordance with Driftwood Policies and Procedures.
- Ensures timeliness and encourages participation of residents in group sessions, individual therapy, outside appointments, and other events as dictated by their schedule.
- Supports staff with the enforcement of and accountability for the rules and guidelines as outlined in the Resident Handbook.
- Assists with inventory, general maintenance of supplies and equipment and maintains a clean and safe environment for residents which includes but is not limited to bed checks, room sweeps, breathalyzing, inventory, and ensuring readiness of units/rooms in accordance with Driftwood Policies and Procedures.
- Monitors client’s self-administration of medications.
- Participates in continuing education activities, remaining knowledgeable in areas related to the focus of the residential programs.
- Assists in making daily visual inspections of property for documenting quality improvement needs, guest or safety concerns, and supplies needed.
- Responsible for knowing and facilitating emergency and safety practices in accordance with Driftwood Policies and Procedures.
- Responsible for maintaining professional boundaries and understanding and following all Driftwood Policies and Procedures.
- Assists staff and residents in the administration and performance of urine analysis and breathalyzer testing in accordance with Driftwood Policies and Procedures.
- Performs pre and post shift inspection of company vehicles with regards to maintenance, safety, and the vehicle’s readiness to operate in accordance with Driftwood Policies and Procedures.
- Communicates concerning client behavior to appropriate staff in a manner which staff fully understands. Assists in crisis intervention as appropriate in accordance with Driftwood Policies and Procedures.
- Interacts with and supervises residents during offsite activities in a manner which protects resident confidentiality and supports resident care.
- Performs other duties as assigned by the Manager of Care Coordinators.

- Oral and Written Communication- The individual speaks clearly and persuasively in positive or negative situations, demonstrates group presentation skills and conducts productive meetings. Is able to read and comprehend simple instructions, correspondence, and memos. Has the ability to write simple correspondence, and effectively present information to individuals, small groups, customers, clients and other employees of the organization.
- Math Reasoning- Ability to add, subtracts, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent to draw and interpret bar graphs.
- Computer Skills- Has knowledge of word processing software, spreadsheets, and Google Suite platform.
- Reasoning Ability- Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.
- Safety and Security- The individual observes safety and security procedures and uses equipment and materials properly.

Qualifications
Preferred Education/Experience:
- One year working experience in the treatment or related field.
- Current CPR and First Aid certificate
- Current TB Test
- Valid Driver’s License
Salary : $20 - $25
