Overview: The Physical Therapist - Home Health is responsible for delivering high-quality physical therapy services in patients' homes to enhance their physical and cognitive abilities. This role involves assessing patients' physical conditions, developing individualized treatment plans, and providing therapeutic interventions to help patients achieve optimal recovery and independence. The Physical Therapist will collaborate with patients' families, provide education on self-care techniques, and maintain detailed clinical documentation. This position requires a proactive approach to patient care, strong communication skills, and the ability to adapt treatments based on ongoing evaluations.

Position: Physical Therapist - Home Health

Responsibilities

• Assess patient functioning levels and develop treatment plans to improve independence and physical/cognitive functioning.
• Provide therapy instructions and adaptive equipment guidance to patients and family members.
• Conduct cognitive and functional re-evaluations and adjust treatment plans as needed.
• Maintain clinical notes and regularly report progress to attending physicians.
• Offer consultation and guidance to home health team members regarding therapeutic equipment and techniques.
• Participate in staff meetings, in-service training, and continuing education.
  
  

Qualifications

• Education: Bachelor’s degree or higher in Occupational Therapy
• Experience: 1 years in Occupational Medicine; 2 years in Social Work
• Licenses/Certifications: Valid Occupational Therapist, Registered (OTR) in Illinois; Valid Driver's License (DL); American Heart Association Certifications (any of the following): Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS)

Additional Information

• Must have 3 years of experience working as a Mobile Application Developer.
• Must have experience with Agile methodologies and iterative development.
• Must have experience in Java and service-oriented architecture.
• Must adhere to 508 compliance regulations and reporting.
