Case Coordinator
JOB TITLE: Case Coordinator
REPORTS TO: Director of Care Team
Supports: Assisting with developing protocols for clients identified Risks
Salary: $56,000
NATURE AND SCOPE OF JOB:
The Case Coordinator will acquire or write protocols for identified risks in the Risk Identification Tool (RIT) for each client. They will obtain copies of the most current ISP documents, RIT, and any other written protocols from the individual/guardian and Service coordinator. The Case Coordinator will verify that all documents match identifying risks. They will be developing protocols and support documents to address each risk.
RESPONSILBILITIES:
- Review client ISP, RIT and any other incoming support documents
- Developing protocols and other safety plans to address identified risks.
- Update any support documents as necessary.
- Maintain Protocol tracker.
- Assist in training Identified DSPs as needed.
- Developing Implementation strategy plans for each ISP goal
- Attending ISP meetings when invited.
QUALIFICATIONS:
- Minimum of 2 years of hands-on industry experience (DSP/PSW, Managing group home or other related industry experience)
- Minimum of 1 year of Writing Support Plans (protocols, Safety plans, etc.)
- Pass Federal Background check.
- Calm and patient demeaner
- Demonstrated ability to coach others
- Well spoken.
- Good Writing skills
- Knowledge of OAR’s and K-Plan as well as how ODDS procedures work
- Excellent administrative skills both written and verbal.
JOB FUNCTIONS:
The Case Coordinator shall:
- Always conduct themselves in a professional manner
- Promote the highest level of respect and kindness to our DSPs & clients/families.
- Monitor and respond to all communications in a timely manner.
- Work collaboratively with their team and brainstorm solutions.
- Produce/Submit telecommute report when applicable.
- Bring all escalated issues to supervisor before making decisions that would be outside our standard operating procedures.
- Attending company functions and greeting clients.
- Abide by HIPAA rules and regulations.
- Keep and maintain notes on all meetings held that are unscheduled.
- Adhere to all policies and procedures as outlined by DSPC management team.
*DSPC ebbs & flows. Expect changes as Management observes and refines processes.
