What are the responsibilities and job description for the LMSW Team Leader position at DT?
The new Team Leader supervises a team of Social Workers who provides mental health and trauma-informed services to clients needing such services, including assessment, planning, counseling, referral, and monitoring services.

Key Responsibilities
· Oversees the team and reports directly to the DSS and PD.
· Conduct intake and client assessment for services.
· Arrange, refer, and monitor care services and support services.
· Communicate with client’s care takers, relatives, friends, and other support network.
· Write client services reports.
· Develop clinical services plans that improve mental, and emotional health of clients.
· Conduct counseling and therapy services within capability and needs of the client.
· Coordinate with outside counseling and behavioral health or mental health professionals.
· Provide on-going assessment, planning and client monitoring around issues relating to their social needs, trauma, mental and behavioral health.
· Assist with responding to crisis management.
· Create evaluative reports on services.
· Participate in agency and program Q/A process.
· Other duties as assigned by Supervisor.
Minimum Qualifications: LMSQ and at least 5 years of relevant experience or MSW. Experience working with People with health, homelessness, poverty, and cultural/language issues.

Pay: $70,000.00 per year
