This is a salaried position ranging $45,000-$52,000 annually based on experience.

Some of the job duties are the following, but not limited to:
- Identifying and meeting the emotional and social needs of our residents and their families.
- Providing discharge planning services for residents, arranging appropriate referrals.
- Providing information to residents and families regarding finanacial assistance programs (i.e. Medicare, Medicaid).
- Interviewing residents and families as necessary to obtain social history and other needed information.
- Developing and revising care plans.
- Attending interdisciplinary care plan meetings.
- Acting as an advocate for residents and reporting violations of residents' rights.
Requirments:
- Must have a minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and phychology.
- One year of supervised social work experiencein a health care setting working directly with individuals.
