What is a LPN Case Manager?As the LPN Case Manager, you will oversee the implementation of nursing processes for individuals served within a regional area. You will work alongside the RN Case Manager supporting a caseload of approximately 200 individuals, traveling throughout the area to train staff, administer routine healthcare activities, and document reviews and medical issues.

About Us: At Duet (formerly ENCOR), we provide support services to adults with intellectual and developmental disabilities with a focus on empowerment and independence throughout 5 counties in Eastern Nebraska. We assist in the activities of daily living, provide community engagement opportunities, job coaching and life skills training, as well as skilled nursing care as needed.
A Day in the Life:
- Flexible Schedule (Monday through Friday and on-call as needed)
- Schedule nursing reviews ensuring the needs of the individuals are met
- Audit medical documentation completed by the Direct Support Professionals and Managers
- Administer routine healthcare activities and special care procedures
- Document reviews and medical issues, reporting any change of statuses to the RN Case Manager
- Act as a medical liaison and medical consultant
- Travel up to 50% of the time throughout the specified region (North/Central Omaha) checking in on those served
- Review appointment records, labs, and diagnostic tests
For the safety of those we serve, we ask the following:
- You possess a strong work ethic
- You have 2 years of experience working as an LPN
- Experience with case management preferred, but not required
- Possess a valid LPN license in the state of Nebraska
- Experience working with the ID/DD population
- Knowledge of or experience with Therap preferred
- Must be willing to undergo a background check, complete a physical, and a drug screen
