Outreach Case Manager- Community based, "boots-on-the-ground" position working closely with the SUD & homeless population. Spend time supporting clients on our RV mobile unit, drive our outreach van and RV, work on the RV engaging in outreach with our community members throughout Cape Cod.

Qualified Candidate- A candidate for this position must have a Masters Level education and/or LADAC and at least two (2) years of experience in case management, preferably working with the homeless/at risk population; or an equivalent combination of education and experience.

Statement of Duties- Under the direction of the Director of Substance Use Disorder Services and Outreach Programs, the Outreach Case Manager II is responsible for acting as a bridge between the community and other community service providers. This person is a trusted worker who is respected by the people they serve and applies their own unique understanding of the experience, socio-economic needs, language, and/or culture of the communities served.
- Working as part of a team to support and sustain community-led action around issues related to our services;
- Working as part of a team to design and deliver a range of innovative community outreach and engagement activities;
- Developing and sustaining positive relationships with a range of people, including residents, community leaders, local groups and organizations, and local authorities;
- Supporting face-to-face and online activities;
- Working in different environments (office-based, mobile unit and on the ground);
- Maintain case notes and participant files and ensure that all reporting requirements are adhered to as outlined by the funder and program for every client interaction;
- Work collaboratively with community partners, and DHC Departments/Staff in linking clients with needed services.
- Support marginalized community members with access to housing resources and decreasing barriers to support services
Duffy Health Center is a patient-centered medical home, and every role requires functioning effectively using a team-based-approach. The Case Manager II is responsible for coordinating the care of patients with providers and outside agencies to provide support, guidance, and advocate for the needs of Duffy Health Center clients. Work includes assessing client needs, developing a service plan, providing interventions, advocating for clients, coordinating with community groups, state and federal agencies, and local organizations to assist clients in obtaining appropriate social services, resources, and assistance.
Education and Experience
A candidate for this position must have a Masters Level education and/or LADAC and at least two years’ experience working with the targeted population, (see Addendum to Job Description)
Licenses and Certifications Required
A candidate for this position must have a valid driver’s license and own transportation, Masters Level license and or LADAC.
Knowledge, Skills and Abilities- A candidate for this position should have:
Knowledge of:
· Resource availability, service costs, and budgetary parameters
· Local groups and organizations providing social services
· Case management practices and procedures
· Issues related to homeless and at-risk individuals, including: mental illness, substance abuse, criminal history, and physical and sexual abuse
· Protocol for handling crisis situations
· HIPAA regulations
· Trauma informed care
· Motivational interviewing.
Skill in:
· Assessing and addressing the needs of homeless and at-risk individuals
· Monitoring and managing client care
· Handling multiple priorities simultaneously in a timely manner
· Planning and problem solving
· Collecting and recording information and preparing reports
· Effective written and verbal communication.
Ability to:
· Be fiscally responsible in carrying out all case management functions and activities
· Have sensitivity for a population who has experienced trauma
· Maintain confidentiality of information
· Interact effectively with homeless and at-risk individuals

Duffy Health Center offers 2 bonus opportunities in the first year of employment. $500 after your 6 month work anniversary. $1000 after your one year work anniversary.
