Job Title: Connected Care Coordinator
Reports to: Clinical Manager, Value Based Care Status: Full-Time, Non-Exempt
The Connected Care Coordinator (CCC) is often one of the first points of contact between patients and Duke City Cares. The Connected Care Coordinator's primary purpose is to act as a liaison between patients and clinics in coordinating chronic care. The Care Coordinator will provide primarily support via technology in completing patient outreach for patients enrolled in our chronic care management program.
Job Responsibilities:
- Answer text messages from patients on company provided software within a 24-hour period
- Responsible for scheduling appointment on behalf of patients
- Responsible for notifying providers with information given or needed by patients
- In charge of filling, scanning, and record keeping of patient data
- Oversees the review process for patient charts
- Maintaining communication with the team
- Maintaining and protecting patient confident
- Answering non-medical patient questions and providing information
- Answering calls and taking messages, Checking voicemail messages
- Performing other duties as assigned
Education, Experience, and Licensing Requirements:
- High School Diploma
- Medical Assistant certification required
- Minimum of 1 year experience in customer service or the medical field preferred
- Previous experience with medical insurance verification preferred
- Basic computer skills. Types 25 words per minute with data entry accuracy
- Pass criminal background check drug screen
Physical Demands Requirements:

- Minimal Lifting, bending and stooping, sitting for 6-8 hours a day. Walking or standing up to 2 hours a day, continuous hand and wrist movement for data entry

- Ability to multitask and prioritize appropriately while maintaining appropriate organization
- Attention to detail in preparing and conveying reports as necessary
- Understanding and verification of medical insurance

- Familiarity with basic medical terminology
Job Type: Full-time
Pay: $16.00 - $20.00 per hour
