Job Title: Medicare Wellness RN
Reports to: Medicare Wellness and CCM Supervisor
Overview/Position summary:
The Medicare Wellness RN (MWN) performs Medicare Wellness visits with identified patients in person or via telehealth, and collaborates with providers for documentation, review, and sign off completion. The ideal candidate for this position must possess compassion, participate in active collaboration and communication, and must perform appropriate clinical judgement in patient care. The MWN must be educated on chronic disease management and preventative care, providing related teaching to patients as needed. They will conduct patient chart reviews for pre-visit planning and other clinical requests, and are responsible for conducting outreach, scheduling, and verification activities for preventative services and Medicare Annual Wellness visits.
The MWN may assist with other clinical duties such as referrals, patient messages, prescription refills etc. They may address open quality care gaps with patients and works collaboratively with patients towards closure of those gaps. They will complete appropriate documents within the electronic record to satisfy closed quality care gaps.
Summary of Primary Clinical Duties:
- Reports to work on time and prepared to begin working at the start of shift
- Conducts thorough and respectful communications with providers
- Reviews Medicare lists/daily schedules of providers to pull MWV
- Relays patient concerns to the appropriate providers during wellness visit
- Schedules Medicare Wellness visits via Zoom, Telemedicine, or in-clinic
- Completes Medicare wellness exams with the assistance of clinical assistant, including but not limited to vitals, eye exams, hearing exams, EKG, and other tasks as required
- Mails NM MOST to patients
- Completes additional administrative duties, including but not limited to faxing, scanning, entering referrals, ordering labs, and communicating by phone with patients or care team
- Helps with triaged chronic care management and remote patient monitoring patients as needed
- Other duties as assigned
Education and Experience:
- Current New Mexico Licensed RN
- Two years of experience in primary healthcare or related setting
- CPR Certification required
Work Environment:
- Frequent interaction with a diverse population, including team members, providers, patients, insurance companies and other members of the public
- Work hours subject to office schedule to ensure coverage during hours of operation
- Participate in regular staff meetings and quality improvement projects to improve patient care
Physical Demands:
- Frequent sitting, standing, walking, grasping, and speaking
- Occasional reaching, bending, and stooping, Lifting, carrying, pushing, and pulling up to 60 pounds
- Frequent use of computer, keyboard, copy and fax machine and phone
- Travel to alternate provider offices to preform MWV Occasional travel to attend meetings or trainings

Pay: $27.00 - $35.00 per hour
