Job Summary
The Geriatrics service at Duke is actively seeking an Advanced Practice Provider (PA / NP) to support the HOPE (Health Optimization Program for Elders) and POSH (Perioperative Optimization for Senior Health) programs at Duke University Hospital. This provider must have a strong commitment to optimizing care for older adults.

Job Duties and Responsibilities :
- Provide direct patient care (daily rounding, performing new consults, etc) under Geriatric Medicine Physician supervision in both inpatient (70% of job duties) and outpatient (SNF visits and outpatient clinic = 30% of job duties). Offsite patient evaluations in the skilled nursing facility setting is also required.
- Perform physical exams, order diagnostics, and prescribe treatments.
- Coordinate care transitions from acute care to SNFs, including medication reviews, risk stratification, and family / patient education.
- Address physical, psychological, and environmental factors impacting successful rehabilitation.
- Collaborate with Geriatrics and Palliative Care teams, ensuring safe transitions.
- Communicate with SNF providers at discharge and provide follow-up within 72 hours.
- Participate in geriatric workgroups, NICHE (Nurses Improving Care for Healthsystem Elders) initiatives, and strategic planning.
- Contribute to exceptional patient care, research, and teaching as part of a dynamic and supportive team.
Job Eligibility Requirements
Education
NP : Graduate of DNP or MSN Nurse Practitioner program (ANP, FNP, ACNP all accepted).
PA : Graduate of ARC accredited Master's Degree or Doctoral Degree Physician Assistant program
Certification
NP : NP certification (ie : ANP, AGNP, AGPCNP, ACNP, AGACNP, FNP)
PA : PA-C
Licensure
NP : Current unrestricted RN license issued by or recognized by the NCBON with authorization to practice as an NP.
PA : Current unrestricted medical license issued by NCMB with authorization to practice as a PA.

Job Hours
FOUR ten-hour shifts per week. Hours are 7am-5pm. No weekend or on-call coverage requirements. Schedule may be changed by administrative or clinical leadership to meet department needs.
