Job Summary
We are seeking an RN Care Coordinator in our Dermatology Department located in Durham, NC. The RN Care Coordinator (RCC) is a professional RN with service line specific clinical knowledge who offers individualized assistance across the care continuum to patients, families, and caregivers to expedite and coordinate care, and address health system barriers. The Dermatology RCC assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes with a focus on high risk patients or those with complex care needs (including multidisciplinary care). The RCC has a particular focus on improving transitions in care from Teledermatology to in-person appointments via e-comms and other urgent referrals. Additionally, the RCC functions as a clinical advocate and educator for patients, assisting with complex medication support through prior authorizations and patient assistance programs.
The RCC will advance the mission of ensuring the right patient has access to the right provider at the right time, and will work with the edit to refine our decision tree and scheduling algorithms to promote access. The RCC will build community relationships with Dermatology’s Clinically Integrated Network to provide expedient and reliable access to quality care. The RCC identifies and alleviates stressors during transitions and removes barriers to care with the aim to deliver a seamless model of care that benefits patients, family members, providers, and the healthcare team.
Job Duties And Responsibilities
The schedule for this position would be business hours, Monday-Friday with no evenings, weekends, holidays or call. This position will be onsite for the first 6-9 months with the option to move to a hybrid schedule at a later date with manager's approval.

Job Duties And Responsibilities
- Assess and facilitate coordination of multidisciplinary care across the care continuum. Provide patient support and education through patient advice request via MyChart. Deliver asynchronous care, communication, advocacy, and education to facilitate care coordination or care escalation.
- Ability to clinically triage new patient referrals efficiently, to ensure timely access to care with e-visits, video visits, nurse visits or urgent appointments with a clinician.
- Collaborate with multidisciplinary care team and expedite sequence of patient's clinical care pathway from E-comms to virtual care, and virtual care to in-person.
- Provide communication and reinforcement of clinical information and serve as a conduit between patient and providers/clinicians to address needs of patients.
- Follow-up and targeted assessment for continuity of care with skin cancer referrals for surgical management with a proceduralist, Mohs or Melanoma Clinic (Cancer Center)
- Assess health literacy and educational needs of patient. Provide clinical education about diagnosis, treatment, side effects, and post- treatment care.
- Assess and document patient's needs, goals, and preferences and work to ensure such are integrated into treatment and care delivery.
- Bridge gaps in care, and assess/troubleshoot care transitions and barriers to care.
- Work with edit team to optimize the decision and scheduling algorithm. Promote 100% decision tree utilization across all sites.
- Facilitate urgent scheduling overflow appointments from the scheduling center to the clinics
- BSN required
- 3 years of RN experience required
- Certified Dermatology Nurse and/or MSN strongly preferred
- Three years of experience of specialty specific nursing in a hospital, clinic, or community setting.
- Experience in nurse navigation preferred.
- Completion of national nurse navigation training preferred.
- Metric tracking skills
- Strong knowledge of the dermatology specialty.
- Experience with nurse triage and patient throughput preferred.
- Must have current or compact RN licensure in the state of North Carolina. BLS required.
