Overview
Manager, Care Ally / Care Management
Full-Time, 40 hours per week.

Responsibilities
Provide direction and manage daily operations of the Care Ally / Care Management Team and act as a clinical resource.
Oversight of the Care Ally / Care Management team, including interviewing and hiring employees and ensuring employee training needs are met.
Conduct staff management activities, including performance evaluations; skill and career development activities; policy administration; coaching on performance management and behavior; employee relations and cost control.
Meet regularly with direct reports to review cases and provide coaching if applicable.
Develop, implement, and update educational resources to support the needs of the staff and clients.
Oversee staff who serves as a patient advocate to ensure timely resolution of issues dealing with complaints and inquiries.
Collaborate with leadership, EMR teams and analytic teams to develop, implement, analyze, and deploy needed reporting to ensure accurate measurement of employee workflows and outcomes.
Collaborate with leadership to determine and achieve performance metrics and targets.
Collaborates with the Director, Transitions of Care to develop and operationalize strategic plans for the Care Ally / Care Management department.
Serve as a liaison between Duly and key stakeholders to promote, improve and communicate initiatives related to Care Ally / Care Management.
Analyze data to identify process and outcome trends, collaborate on population risk stratification for purposes of allocation of care ally / care management resources, modify care ally / care management activities accordingly.
Assist in defining reporting requirements for generating measure-related reports and collaborates with necessary enterprise-wide business areas to create measure reporting dashboards.
Collaborate with enterprise-wide business areas to improve systems / processes for gathering, managing, and reporting Care Ally / Care Management data.
Serve as a liaison with regulatory and accrediting agencies to ensure compliance with standards, as applicable.
Participate with contracted managed care organizations as necessary to ensure compliance with health plan and regulatory standards, as applicable.
Continue to evolve care ally / care management programs with a focus on developing and implementing necessary tools and programs related to care planning and addressing social determinants of health.
Qualifications
Current Unrestricted Illinois RN License
Current CCM certificate, preferred
Experience with Medicare populations, preferred.
5 years relevant work experience, Minimum 3 years of medical / surgical or ambulatory experience in a healthcare setting. Preferably Value-Based Care experience
Proven track record of delivering operational results in a matrixed environment
Excellent communication and interpersonal skills with ability to influence and inspire others
Knowledge of value-based care principles, healthcare operations, regulatory requirements
Project management expertise and ability to foster collaboration across diverse teams
Advanced Microsoft office suite skills.
The compensation for this role includes a base pay range of $99,847- $149,770 with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
