This position will conduct outreach and education services specifically for individuals who have opioid or stimulant substance use disorders and/or have experienced or are at high risk of experiencing an overdose and works with a team of health care professionals to provide clients with opportunities to manage and improve their health by facilitating care coordination through counseling or medical assisted treatment, enhancing access to community-based services, providing addition education, and addressing social determinants of health. This individual will work with individuals as part of an integrated team of health care providers.

Significant travel is required. Up to 50% of the time, this team member will be in the field making home visits and working with community-based agencies. Work is performed in the office area and remotely in other locations to provide flexibility for meeting with clients, community partners, vendors, etc. within Stephens County in Southwest Oklahoma. This position is grant-funded.

RESPONSIBILITIES (ESSENTIAL FUNCTIONS):

  • Develops and maintains collaborative partnerships with health systems, community clinics, healthcare providers, and other community partners.
  • Supports patient engagement and recruitment efforts for substance use treatment.
  • Maintains accurate county and community data for service area, diligently recording all encounters in grant database.
  • Supports and encourages policies, workflows, and collaborations that remove/reduce inequities, administrative barriers, stigma, and other obstacles that impede access to and retention in evidence-based care for Opioid use and stimulant use disorders.
  • Identifies and implements ways to build health system capacity for care coordination/warm hand-offs/trauma-informed longitudinal care for opioid use disorder and stimulant use disorder.
  • Utilizes fatal and nonfatal overdose data to tailor health system engagement and increase focus on populations who are at disproportionate risk of overdose.
  • Supports clinician training by providing and promoting education on substance use disorders, addiction-related stigma, overdose prevention, and harm reduction best practices.
  • Collects and reports to the Injury Prevention Service the required data measures at least quarterly.
  • Provides monthly reports with each invoice documenting progress made on health system engagement, programmatic development and strategies, and linkages to care.
  • Assists community members with barriers to care coordination access; assists with connecting members to community partners as needed.
  • Tracks and reports client encounters and maintains required records as directed. Maintains appropriate confidentiality of information.
  • Acts as a patient advocate.
  • Provides client follow-up and monitoring, including phone calls and home visits with patients as needed.
  • Demonstrates knowledge and support of DRH Health's mission, vision, value statements, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
  • Actively participates in required training and professional development activities.
  • Maintains professional and technical knowledge through continuing education opportunities including internal and external educational offerings.
  • Regular attendance and punctuality for scheduled shifts.
  • Utilization of assistive devices for lifting is mandatory.
  • Must adhere to safety protocols at all times.
  • Per DRH policy, all required conditions of employment must be met and maintained including required vaccinations.
  • Implement DRH Standards of Behavior and exhibit behaviors consistent with DRH core values.
  • Performs other related duties as assigned.

Minimum Qualifications: Communication skills including fluency in oral and written English. Basic computer skills including the ability to send/receive/email, navigate information technology associated with the position, and use Electronic Health Record information tools. Excellent written, verbal, and interpersonal communication skills; Ability to adapt procedures, processes, tools, equipment, and techniques to accomplish the requirements of the position. Advanced, fingerprint background checks are required for this position.

Works effectively in team environment, participating and assisting their peers. Ability to organize, coordinate, and demonstrate judgement, professionalism, diplomacy, assertiveness, and initiative. Frequent departmental interfaces with internal and external customers to coordinate and provide optimal level of program services. Knowledge and ability to develop and implement policies and procedures. This position requires effective time management skills, ability to comply and operate independently within organizational policies and/or direction of supervisor.

Education and/or Experience: High school diploma or equivalent. At least two years of work experience in addiction treatment, recovery, or peer counselor role. Excellent skills in using Microsoft Word and Excel and Google Documents.

Certifications, Licenses, Registrations: A current, valid drivers license and automobile liability insurance must be maintained. Must have reliable transportation.
