LPN - Home Health Nurse
We are seeking a dedicated and skilled LPN to provide general nursing care and related services for assigned patients in a home health setting. Under the direction of a Registered Nurse (RN) or Medical Doctor (MD), you will follow established policies to deliver quality patient care. You will be responsible for performing specialized procedures, including sterile dressings, irritations, and cauterizations, while utilizing your knowledge of asepsis. Additionally, you will offer functional direction to ancillary personnel and actively participate in the planning and provision of patient care.
Key Responsibilities:
- Perform general nursing procedures and specialized treatments under the supervision of an RN or MD.
- Ensure proper aseptic techniques are followed when providing treatments, such as sterile dressings and cauterizations.
- Provide functional direction to ancillary staff, assisting with the coordination of patient care.
- Collaborate with the healthcare team to create and implement patient care plans.
- Travel to assigned patient homes to deliver care and services.
Qualifications:
- 1 year of Home Health experience – Required
- Travel experience – Required
- Active and clear Driver's License and Car Insurance – Required
- Hawaii LPN license – Required
- Experience with Homecare Home Base and OASIS – Required
- COVID and Flu vaccination – Required (Declinations will not be accepted)
- Arrhythmia certification – Preferred
- BLS certification – Required
Additional Information:
- Mileage reimbursed at $0.43 per mile.
State License Requirements:
- Must hold an active Hawaii LPN license.
