Position: LPN - Long-Term Care (LTC)
Position Summary:
Under the direction of the RN Supervisor, the LPN Wing Supervisor will be responsible for implementing the medical plan of care and supervising the Nursing Assistants (NAs) in providing care to residents and students, ensuring that care is delivered in accordance with best nursing practices and applicable State and Federal regulations. The ideal candidate will have a passion for working with residents daily and be committed to fostering a compassionate and supportive environment.
General Duties:
- Administer and record medications, treatments, and monitor vital signs (e.g., glucose checks) as ordered; report and document resident response.
- Accurately transcribe physician's orders.
- Complete monthly summaries and review the care plan with NAs to ensure it reflects the current condition of the residents.
- Notify the RN Supervisor immediately of any significant changes in a resident's condition and report updates as necessary.
- Communicate transfers of residents to immediate family members and via email to department heads, ensuring proper documentation and transfer forms are completed.
- Assist physicians with rounds, examinations, and treatments as needed.
- Collect laboratory specimens (urine, cultures, stool, etc.) and arrange for delivery to the hospital with accurate requisitions.
- Supervise post-mortem procedures upon a resident's death, assist with collecting clothing and belongings, and notify department heads accordingly.
- Encourage resident participation in scheduled activities and monitor their involvement.
- Assist with evaluations and performance reviews for nursing assistants, providing feedback and counseling as needed.
- Maintain accurate documentation of weights, vital signs, and follow up on discrepancies.
- Clean and stock treatment trays daily and maintain work areas in a safe and organized condition.
- Submit maintenance requests and monitor safety hazards in the environment.
- Provide direction to nursing assistants to ensure the prescribed care plan is followed and implement any necessary changes.
- Perform additional duties as directed by the supervisor, including attending care plan conferences and assisting with the orientation of new nursing staff.
- Adjust staffing assignments as needed to ensure quality resident care and oversee the team’s compliance with infection control practices.
- Provide direct care to residents as needed within your scope of practice.
- Document narcotic drug disposal on resident charts and participate in departmental meetings and trainings as required.

- Jewelry: No large or dangling earrings or necklaces

- Name tags: Must be worn daily
- No short or jogging pants
Physical Demands:
- Good physical health and mental alertness to perform job functions efficiently
- Ability to pass employment physicals and meet physical demands of the job
- Must be free of communicable diseases
- Must be able to lift, carry, and transfer residents with proper body mechanics

- Phone screen with CEO
- Video interview with HR, DON, ADON, and CEO (via Zoom)
- Interview with team members (via Zoom)
Submission Requirements:
- Nursing home experience is a plus, but new grads are welcome to apply.
- BLS (AHA) certification required.
- COVID vaccine preferred but not required.
- Must possess a current Pennsylvania LPN license or have applied for a temporary work permit at the time of interview.
- Must be knowledgeable in medical asepsis, body mechanics, and personal care of residents.
- Demonstrated ability to follow written and verbal directives and communicate effectively with residents, visitors, and coworkers.
Certification Requirements:
- BLS (AHA)
State License Requirements:
- Pennsylvania LPN license required.
Shift Notes:
- Nights: 6:00 PM - 6:00 AM
- Schedule: Two days on, two days off, three days on (84 hours biweekly)
