Social Worker

Essential Job Functions/Responsibilities

• Performing comprehensive patient psychosocial assessments of patients and families.
• Assist in the development and implementation of the patient/family Plan of Care
• Complete the Initial Psychosocial Assessment within 5 calendar days of admission.
• Coordinating community resources for patients and families.
• Provide social service activities as related to terminally ill patients and their families, including planning, treatment and consultation with other staff.
• Provide counseling to patients/families and hospice staff as needed.
• Helping family members cope with the loss of a loved one through bereavement support visits, phone calls, and support groups.
• Providing patients and families with emotional support through their hospice journey through in home or in facility visits.
• Helping guide patients and families through difficult conversations and decisions about end of life care such as funeral home preferences, code status, and other discussions about a patient’s last wishes.

Position Qualifications:

• Masters in Social Work.
• Previous experience in hospice is preferred. Proven ability to work within an interdisciplinary setting preferred.
• Complies with accepted professional standards and practice.
• Has excellent observation and communication skills. Understands hospice philosophy and issues of death/dying.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Willingness to travel throughout assigned service area.

Job Type: Full-time

Pay: $65,000.00 - $72,000.00 per year
