This includes facilitating communication between clients and service providers, coordinating treatment plans, and ensuring client rights are respected.

* Coordinate treatment plans and services for clients* Facilitate communication between clients and service providers* Ensure client rights are respected and protected* Collaborate with other staff members to provide comprehensive care and services* Maintain accurate records and reports* Attend training programs and staff meetings* Perform other duties as assigned*
