RN or LPN home health care

Job Overview
We are seeking compassionate and dedicated Registered Nurse (RN) or Licensed practical nurse (LPN) to join our Home Health team. The ideal candidate will be responsible for providing high-quality nursing care to the patient in their home, ensuring their health and well-being while promoting independence and comfort. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care.

Responsibilities

• Conduct comprehensive assessments of patients' health status and needs in their home environment.
• Administer medications and treatments as prescribed and other specialized procedures.
• Monitor patients’ progress and adjust care plans as necessary to achieve optimal health outcomes.
• Educate patients and their families on managing health conditions, medication administration, and preventive care strategies.
• Maintain accurate documentation of patient care activities in accordance with regulatory standards.
• Provide support in urgent care situations and emergency medicine as needed.

Qualifications

• Current (RN) or (LPN) license in the state of Ohio.
• Experience in home health care or related outpatient settings is preferred.
• Strong clinical assessment skills with the ability to work independently in a home setting.
• Excellent communication skills for effective interaction with patients, families, and healthcare teams.

Job Types: Part-time, Per diem

Pay: $27.00 - $35.00 per hour

Expected hours: 8 – 28 per week
