🕘 Full-Time | Monday–Friday | 9 AM – 5 PM
We’re looking for a dedicated Registered Nurse (RN) with a passion for care coordination and resource management to join our team in the Bronx! If you're experienced in case management and ready for a steady weekday schedule, we want to hear from you.
💰 Pay Package:
- Gross Weekly: $3,400
- Non-Taxable Stipend: $2,600/week
- W2 Rate: $20/hr
🩺 Responsibilities:
- Manage patient cases from admission through discharge, ensuring effective and efficient care coordination
- Collaborate with interdisciplinary teams to develop care plans and connect patients to appropriate resources
- Review utilization and eligibility to support quality outcomes and cost-effective care
- Maintain accurate documentation using EPIC
- Conduct PRI and SCREEN assessments (preferred)
✅ Requirements:
- Active New York State RN License
- AHA BLS Certification (Basic Life Support)
- 2 years of experience in case/resource management and utilization management
- 3–5 years of experience in care management preferred
- Experience with EPIC strongly preferred
- PRI and SCREEN certification/experience is ideal
