Job Overview
The Bilingual Case Manager plays a crucial role in delivering Home-based Crisis Intervention and comprehensive case management services to client families engaged in the Alternatives for Youth Program. You will assist both Spanish and English-speaking families in overcoming challenges such as substance abuse and mental health issues, guiding them toward stability and resilience.
Key Responsibilities
- Provide crisis management support to families facing difficulties that have led to PINS petitions.
- Deliver intensive, home-based case management services aimed at restoring family stability.
- Conduct assessments using the Child and Adult Needs Assessment (CANS) tool and formulate Family Intervention Plans tailored to family needs.
- Refer families to appropriate partner agencies and proactively engage with new service providers when necessary.
- Monitor family progress by following up with service providers and maintaining robust case records in compliance with contract requirements.
- Assist parents and youth in acquiring behavior management techniques.
- Participate in team discussions, case conferences, and ongoing training sessions.
- Facilitate educational workshops and recreational activities for adolescents as directed by the program director.
- Be available on-call to respond to families reaching out via the AFY hotline, as scheduled.
- Strong communication capabilities in both English and Spanish, both oral and written.
- Proficient in using computer applications including Microsoft Office and database software.
- Demonstrated ability to work effectively with adolescents and families in crisis situations.
- Excellent problem-solving and interpersonal skills.
- A Bachelor's Degree is required, along with at least one year of experience in child and adolescent services or related community/social service work.
- Commitment to participate in a minimum of 20 hours of training within the first six months of employment, followed by an additional 20 hours in the subsequent six months on relevant topics.
- A valid New York State Driver's License and access to a vehicle are necessary for this role.
