Key Responsibilities
- Serve as a liaison to the courts and correctional institutions, facilitating case-finding and screening services to develop suitable service plans for participants.
- Implement a thorough screening process to ensure effective referrals to treatment and assist clients with transportation to treatment facilities when needed.
- Submit timely monitoring reports to the MMHC Resource Coordinator prior to scheduled court appearances to outline participant compliance with treatment plans.
- Immediately inform the court of any significant infractions committed by participants.
- Maintain accurate and accessible case documentation, including client records and progress notes, to assist in client support and fulfill program requirements.
- Support clients in applying for public and private benefits and monitor their engagement in treatment programs through regular case management appointments.
- Conduct regular visits to treatment facilities and maintain comprehensive logs of these visits.
- Prepare and deliver both written and oral status reports to the criminal justice system regarding client progress.
- Ensure consistent contact with program participants and treatment providers at an appropriate frequency.
- Participate in case conferences and investigate supportive services or entitlements for clients as necessary.
- Conduct supervised toxicology tests on participants as required.
- Assist the Program Director with various administrative tasks associated with program management.
- Bilingual fluency in Spanish.
- Excellent oral and written communication skills.
- Ability to collaborate effectively with treatment providers and the criminal justice system.
- Proficiency in computer applications, including Microsoft Office and database software, needed for effective task execution.
- A Bachelor's degree is required; a Master's degree in a relevant field such as criminal justice, social work, counseling, forensic psychology, psychology, sociology, or criminology is preferred.
- Minimum of one year of experience working with criminal justice, substance abuse, or mentally ill populations.
