Program Overview
The Suffolk Transitional Case Management Substance Use Disorder program aims to support individuals with substance use disorders in achieving long-term recovery. Our team works collaboratively with case managers, program participants, and community organizations to provide comprehensive services that address the unique needs of each individual.
As a Certified Recovery Peer Advocate, you will play a critical role in this effort. You will work closely with participants to develop personalized recovery plans, connect them with community resources, and provide ongoing support and guidance.
This role requires a strong understanding of the substance use disorder recovery process and the ability to work with diverse populations. You will also need excellent communication skills, computer literacy, and the ability to maintain confidentiality.
