Salary Range: $41-55/hour

Reports to (please list title, not person): Community Health Manager

Schedule: 40 hrs/week, M-F generally

Job Summary: The Community Health Nurse performs assessments, treatments, and collaborative case management of  home health services and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.




Essential Responsibilities:

  • Develops and initiates the plan of care in collaboration with an interdisciplinary team and attending physician. This includes collaboration with patient and/or family

  • Communicates regularly with the physician who is responsible for the home health plan of care and other health care practitioners (as appropriate) related to the plan of care

  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient

  • Provides services requiring substantial and specialized nursing skill, in accordance with the plan of treatment signed by the physician 

  • Demonstrates competency in all skills required for the agency, including, but not limited to, venipunctures, infusion therapy, infusion therapy device care, wound care, and aseptic technique

  • Performs patient assessments based on agency standards of practice and scope of protocols to ensure effective and appropriate home care services

  • Evaluates, and regularly reevaluates, the nursing needs of the patient; initiates, develops, implements and makes necessary revisions to the patient’s plan of care.

  • Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate to the patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so

  • Coordinates and collaborates on the plan of care and maintains continuity of patient care by liaising with other health professionals assigned to the same patients

  • Documents patient care in an efficient and professional manner

  • Participates in the agency’s quality improvement program

  • Represents the agency in collaborative community discussions and meetings relative to home health services

  • Supports home health administrative functions such as regulatory checks, quality reviews and EHR management

  • Participates in on-call coverage as needed

  • Other duties as assigned

  • Expected to protect the privacy of all patient information in accordance with the District’s privacy policies, procedures, and practices, as required by federal and Colorado law, and in accordance with general principles of professionalism as a health care provider. 

  • May access protected health information and other patient information only to the extent that is necessary to complete job duties. May only share such information with those who have a need to know specific patient information to complete their job responsibilities related to treatment, payment or other company operations. 

  • Encouraged and expected to report, without the threat of retaliation, any concerns regarding the District’s policies and procedures on patient privacy and any observed practices in violation of that policy to the designated Privacy Officer. 

  • Expected to actively participate in District privacy training and is required to communicate privacy policy information to coworkers, students, patients and others in accordance with District policy.

Essential qualifications, knowledge, skills and abilities needed for position:

Experience and Knowledge

Knowledge of equipment resources and suppliers for all patient populations. 

Knowledge of therapeutic methods, ability to assess complex patients and determine treatment plans. 

Knowledge of medical terminology, ability to understand impact of particular illnesses upon patient’s physical needs with implications for treatment/care requirements. 

Minimum two years of experience, at least one of which is in the area of public health or home care nursing is preferred

Home Health Experience

Administratively competent and proficient in Google Suite and electronic health records platforms

Ability to complete concise and appropriate case notes and prepare documentation according to established guidelines. 

Self-directed and able to work with minimal supervision



















Licenses or Accreditations

Valid State of Colorado motor vehicle operating license, with continued safe driving history

Current licensure to practice nursing in Colorado (Registered Nurse or Licensed Practical Nurse)

CPR and First Aid certification






Computer Operations

Proficient computer skills; the ability to type, keyboard, and navigate in various programs. 


Language and Communication Skills

Ability to communicate orally with a wide range of individuals, in various and potentially stressful situations. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence with a high degree of professionalism and accuracy. Ability to effectively interact and communicate with patients, co-workers and District staff. 


Reasoning Skills

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, and diagram or schedule form. Must practice sound decision-making skills.


Mathematical Skills

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio and percent.


Language Ability

Bilingual in Spanish a plus
