What are the responsibilities and job description for the Help Me Grow Care Coordinator position at Early Learning Coalition of Lake County?
PURPOSE: To provide service coordination for eligible children and their families by promoting healthy development. This is accomplished by addressing the needs for early identification of development and/or behavioral concerns and linking children and their families to community-based services and supports.
PRIMARY DUTIES AND RESPONSIBILITIES:
Acts as a single point of program contact for carrying out the activities of service coordination by developing in-depth knowledge of Help Me Grow and local systems and services focuses on early childhood development and special health care needs, as well as the eligibility, referral, and application processes for these and related services.
Develop, strengthen, and coordinate collaborative relationships with appropriate organizations and individuals, including the partnership with 2-1-1, including participate in various community meetings and outreach activities as determined appropriate by your supervisor.
Respond to calls from parents, childcare providers, educators, health professionals and social service partners about child development issues and services, as well as facilitating and monitoring the timely receipt of services by families.
Maintain all records within ELCLC and program standards.
Provide crisis intervention when appropriate, assess and connect clients with services through referrals and conference calls, advocate as needed and provide care coordination and follow-up.
Working with ELCLC management team to help identify, document and resolve service gaps and barriers.
Use 2-1-1 and Florida Help Me Grow systems to access information about resources, document calls, collect required data and produce reports.
Maintain First Aid/CPR (adult and child) certifications.
Contribute to a harmonious working environment and support staff in the promotion of ELCLC strategies and goals.
Ensure that all information of confidential nature is held in a secure and confidential manner.
Attend, and assist with, a minimum of two community outreach events annually outside regularly scheduled operating hours of the Coalition, including evenings and weekends.
Attend a minimum of two professional development trainings annually as approved by your department officer.
Other duties as assigned.
Knowledge, Skills and Abilities:
Knowledge of community resources including systems, organizations and programs that deal with children and families facing developmental, behavioral and learning challenges.
Ability to speak, interpret and translate Spanish is a positive asset for the position.
Requires excellent interpersonal skills and the ability to work effectively with people of diverse backgrounds as well as interact with all levels of management.
Must have strong computer skills, including proficiency with Microsoft Office Suite. Knowledge of Statewide Information System preferred.
Well-developed communication skills, both written and oral.
Should be creative, energetic, a self-starter, able to prioritize and work independently with minimal supervision.
Requirements:
While performing the duties of this job, the employee is regularly required to stand; walk; sit for extensive periods of time; use hands and fingers to operate a computer and telephone keyboard and calculator; reach with hands and arms; and stoop, kneel or crouch. In addition, the employee will occasionally lift and/or move up to 50 lbs.

Provide a valid driver’s license and proof of automobile insurance annually. My driving record may be checked, and a record of violations may result in limitations to my ability to drive on ELCLC business or the termination of my employment if driving is an essential job function.
Subject to passing a local and national background screening, including fingerprinting.
Must be subject to drug screening.
Ability to work flexible/hours schedule. Community event hours will be predominately outside
regularly scheduled hours including evenings and weekends.
