- POSITION SUMMARY
- Performs Utilization Management functions to optimize resource use, proper patient status and reimbursement for all admitted patients. Decrease inappropriate admissions, readmissions and ED recidivism by appropriate community referrals, assisting with comprehensive care plans and patient follow-up. Facilitate timely access to community-based services that enhances the health outcomes.
- POSITION QUALIFICATIONS
- Minimum Education
- Associate Degree RN or Bachelor of Social Work
- Minimum Experience
- 2 years nursing or medical social work
- Required Registration/License/Certification
- Active Alabama license by State Board of Social Work/Registered Nursing License
- Preferred Education
- BSN or MSW
- Preferred Experience
- 5 years nursing or social work experience in case management or medical social work
- Preferred Registration/License/Certification
- Case Management Certification from an Accredited Organization
