What are the responsibilities and job description for the Case Manager/SUD Specialist - (Bachelor's/LCDP required) position at East Bay Community Action Program?
East Bay Community Action Program (EBCAP) is seeking a full time Case Manager that holds a Bachelor's degree and holds a LCDP (Licensed Chemical Dependency Professional) for our Behavioral Health Services Division. The successful candidate will perform a variety of case management and care coordination duties relating to the complete case management in support of clients living with Severe and Persistent Mental Illness (SPMI) who are living independently in the community.
How you will make an impact:
- Assist clients to recognize triggers, develop relapse prevention skills, and build support systems.
- Educate clients about the physical, medical, and pharmacological issues relating to substance use.
- Understand and utilize ASAM criteria as part of the treatment process when determining an appropriate level of care for clients treatment.
- Conduct urine tox screens and breathalyzer tests on his/her clients and conducts supervised urine tox screens on court ordered clients as directed.
Your skills & experience:
- Bachelors Degree in Human Services field and (2) years of direct experience required.
- LCDP required
- Must have a valid drivers license, insurance and acceptable driving record if engaged in travel on behalf of the organization.
- Willing to work as needed on a rotating basis for weekends.
