Job Summary
The Community Health Worker (CHW) plays a vital role in bridging the gap between healthcare providers and the community. This position focuses on promoting health education, enhancing access to services, and supporting veterans in navigating the healthcare system. The CHW will work closely with diverse veterans' populations, providing resources and assistance to improve overall community health outcomes.
Duties
Serves as a trusted community liaison between the participants and their families and health providers, VA facilities, community resources and other Veteran resources
Assists participants in navigating the health and human services systems, ensuring appointments are scheduled per the referral process
Connects Veterans and their families to local veteran services officers to enroll in VA benefits. Connects Veterans to nearest VA Health facility and assists with enrollment in VA medical care
Complete CHW and all other required training in its entirety and in a timely manner
Coordinates/facilitates community events targeting Veterans and their families
Identifies, accesses and, when needed, accompanies Veterans to available community resources, supports, and services (e.g., self-help groups, legal services, food bank, housing)
Distributes surveys and collects data.
Provides follow – up with participants via phone calls, home visits and visits to other settings where participants can be found.
Provides connections to outreach activities, education, and baseline mental health screenings within the community to reach veterans in the target counties
Uses social media platforms and other platforms to promote program.
All other duties as assigned that are associated with this position.
Experience
Military experience preferred and/or Experience working with veterans.
Experience working in a community-based setting/health/social services for at least 1 to 2 years preferred.
Basic computer skills required
Proficiency in Microsoft Office
Good organizational skills
Experience with data collection/analysis
Understand the community served and how to foster community connectedness.
Effective communication skills, such as listening well, and using language appropriately.
Ability and willingness to provide emotional support, encouragement, and motivation to eligible participants.
A passion for improving community health and well-being is crucial for success in this role.
Job Types: Part-time, Contract
Pay: $16.00 - $18.00 per hour
Expected hours: 20 – 30 per week
