General Description

Under the supervision of the Executive Director of East Haddam Youth and Family Services, Inc., the Social Worker provides targeted programs and interventions designed to promote positive outcomes and an enhanced quality of life conditions for youth and families in the East Haddam community. Using a client-centered approach, the Social Worker performs confidential work which may include assessment, direct service, case management, crisis intervention, referral and connection with appropriate local and regional programs and services.

Essential Duties and Responsibilities:

The EHYFS Social Worker conducts comprehensive screenings and assessments in support of child, youth and family mental health and wellness; assists with client-led goal setting, provides resources, referrals, and short-term counseling, case management, and crisis intervention when necessary. Provides crisis evaluations both in office and in community and takes appropriate actions to keep clients and others safe. Provides social work support to other town departments such as police and emergency medical services and works collaboratively with the schools and other local, regional, and state providers. Maintains confidential client records.

Juvenile Review Board:

- Conduct intakes and complete intake reports.

- Schedule and facilitate monthly meetings.

- Guide restorative justice interventions.

- Provide regular and on-going follow-up and case management, including:

1) Coordinate with schools, police, local and regional organizations, JRB team members, and families to support youth involved in minor offenses such as misdemeanor crimes, truancy, defiance of school rules, etc.

2) Maintain weekly contact with youth and parents to assist youth in completing diversions set forth by the JRB team.

3) Assist youth and families in finding and connecting with resources.

4) Complete all necessary documentation, including intake and final report, Ohio Scales, and all required statistical data.

Student Support Services:

- Upon referral, provide community support services to East Haddam schools in addressing student-related issues, including attendance, behavior, family, etc. that affect the student’s ability to be successful.

1) Work closely with the Pupil Services Director, school administrators, school social workers, guidance counselors, and other support staff to assess student needs and provide appropriate community-based referrals and services.

2) Participate in regularly scheduled meetings.

Other:

- Parent Support groups

- Youth Support Groups, including LGBTQ

- Maintain a referral network of service providers.

- Data collection and reporting as required.

- Perform other duties as required by the Executive Director.

Minimum Qualifications:

- Bachelor’s degree in social work or related field with 3-5 years’ experience working with children, youth, and/or families.

- Must possess a valid driver’s license.

- A working knowledge of treatment and prevention resources. Knowledge of restorative practices, juvenile diversion, and truancy prevention a plus.

- Proven ability to work independently as well as collaboratively across varying races, ethnicities, and cultures.

- Strong oral and written communication skills.

- Proven ability to establish and maintain effective working relationships with co-workers, other service providers, and the public.

- Strong computer skills with proficiency in Microsoft Office and data collection systems.
