What are the responsibilities and job description for the Adult Nurse Practitioner position at East Harlem Council for Human Services, Inc.?
The Adult Nurse Practitioner position is based at the Boriken Neighborhood Health Center, a state-of-the-art Federally Qualified Health Center (FQHC) and NCQA-accredited Patient-Centered Medical Home in East Harlem offering comprehensive pediatric and adult primary care, specialty care, and the Boriken Pharmacy. Provides telephone coverage, based on the Health Center’s on-call schedule.
RESPONSIBILITIES: Provides comprehensive case managed primary care services in an ambulatory care setting. Contributes to quality assurance, staff development, and other programmatic and clinical functions.
QUALIFICATIONS: Current New York State Nurse Practitioner and Registered Nurse License. NCAA Certified. DEA Certification. CPR Certification. Infection Control Certification. Excellent written, verbal, and interpersonal skills. Two to three year experience in medicine preferred. Knowledge of general nursing theory, practice, methods, and techniques involved in the performance of general nursing services. Bilingual Spanish preferred. Must have Active NYS Medicaid/Medicare Provider #.

Schedule: Full time (35 hours per week, 7 hours per day with an unpaid lunch).
Salary: 135-145k per year dependent on experience.
