What are the responsibilities and job description for the Certified Peer Recovery Advocate position at East Harlem Council for Human Services, Inc.?
RESPONSIBILITIES:Provide peer support to the most vulnerable patients of the Boriken Neighborhood Health Center. Working within a patient centered medical home model, provide substance use case management services, entitlement assistance, concrete service needs, and coordinate referrals to services available through the East Harlem Council for Human Services, Inc. and other agencies. Actively participate in performance improvement and peer review process. Serve as an integral member of a multi-disciplinary care team.
DUTIES:
1. Provides peer support to behavioral health including orientation to program.
2. Provides relapse prevention planning, rehabilitative and recovery-oriented interventions for clients diagnosed with substance use disorders.
3. Provide recovery education and counseling to patients and their family members.
4. Provide coordinated referrals to self-help groups in the community available through the East Harlem Council for Human Services, Inc. and other agencies.
5. Attends and participates in various appointments/meetings to support and advocate for financial entitlements or other beneficial services that promote recovery.
6. Conduct visits with patients after a discharge from a hospital, inpatient rehab stay to facilitate community reintegration and increase treatment compliance.
7. Maintain compliance with all organizational policies and procedures.
8. Actively participate in performance improvement, staff committees and peer review activities.
9. Participate in internal and external meetings and outreach activities of the East Harlem Council for Human Services, Inc. and the Boriken Neighborhood Health Center.
10. Develop and strengthen linkages between other institutions providing services to patients and their families.
11. Adhere to the Council’s policies and procedures and clinical protocols, and other standards of professional conduct. Participate in continuing education to maintain a solid knowledge base through in-service presentations at the Council and other agencies, and through other forms of continuing education.
12. Perform additional relevant duties as requested by supervisor/management.

QUALIFICATIONS:
High School Diploma or Equivalency degree.
Minimum of one year’s experience providing peer services to a diverse population in a social service program preferred.
Knowledge of community-based resources and support services for vulnerable individuals
Highly motivated self-starter, able to translate goals into action and deliver service outcomes
Experience working with patients and families, including individuals with a history of
substance abuse, mental illness, domestic violence and families at risk.
Dedicated to providing high quality patient-centered services.
Bilingual Spanish strongly required.
