Position Title: Social Service Designee

Overview: The Social Service Designee is responsible for ensuring that medically related emotional and social needs of the residents are met and maintained on an individual basis in accordance with Community policies and procedures, and current Federal, State, and Local Standards, guidelines and regulations.

Essential Job Functions:

• Completes initial visit and detailed notes of each resident within 72 hours of admittance.
• Completes required sections of the Minimum Data Sets (MDS).
• Conducts a comprehensive full assessment and care plan (that includes the resident’s needs, strengths, and preferences) within 3 to 7 days.
• Orients residents and families to the facility, services offered, and residents’ rights.
• Schedules and/or attends care meetings and identifies psychosocial needs of residents for inclusion in the care plan; addresses matters such as schedule, treatment, and environment that support a resident’s individuality.
• Documents all information and interactions pertaining to the resident.
• Facilitates any transition due to a change of status such as hospice or insurance coverage. Informs family members, when appropriate, of any resident changes.
• Responsible for DNR process and provides information to each resident on advance directives.
• Facilitates appeal or grievance process when requested by resident or family member.
• Assists residents and families with financial questions and makes appropriate referrals.
• Coordinates visits/appointments with physicians and providers and makes residents referrals.
• Requests doctor’s orders for any equipment or home health services that is needed by a Medicare resident or private resident.
• Develops a comprehensive discharge plan in collaboration with residents, families, multidisciplinary staff, and referral agencies which identifies resident needs and preferences.
• Reports possible cases of abuse to Director of Nursing and is involved in the investigation process.
• Assists the Case Manager in their absence.
• Maintains and promotes good public relations with internal and external community by actively participating in opportunities to promote and market East Ridge Retirement Village.
• Performs other duties as assigned.

You Have:

• Bachelor’s Degree in Social Work or related healthcare field preferred.
• Minimum of 1-2 years relative experience, preferably in a long term care setting.
