Position Title : Memory Support LPN
Overview : The Staff Nurse is responsible for the planning, implementation and evaluation of all nursing care prescribed and provided to Skilled Nursing residents.
ESSENTIAL JOB DUTIES :
The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
- Performs professional nursing services which includes, but is not limited to resident assessments, recording vital signs, administering medications, intravenous fluids, enemas, catheterization, suctioning, sterile technique dressings, setup and administration of oxygen, within their scope of practice.
- Participates in pre-admissions assessments for potential residents.
- Participates in all in-service programs to expand and develop abilities.
- Maintains compliance with all personnel policies, established community policies and procedures, and Federal / State regulations and standards.
- Understands and implements the Fire Safety and Disaster Preparedness Plan and evacuation procedures of the community during drills and emergencies.
- Performs admission and periodic assessments of residents for functional and mental abilities.
- Assists in the development of service plans for residents based on needs assessments.
- Reviews / revises assessments and service plans at change in resident condition and at frequency determined by state.
- Obtains and documents residents' vital signs and weight and intervenes appropriately.
- Assists residents in scheduling medical, dental and laboratory appointments.
- Assists physician with resident examination as needed.
- Reviews and transcribes physician's orders.
- Administers and documents medications in accordance with the five rights : right medication, right route, right resident, right time, and right dose.
- Provides medical treatments as ordered by physicians.
- Supervises and monitors medications for residents who self-administer.
- Observes residents and reports condition changes to physicians, health care proxies, and supervisors.
- Supervises residents in self-care of medical equipment, such as oxygen, catheters, colostomies, etc.
- Prepares or examines food trays for prescribed diets and assists residents with meals.
- Assists residents with activities of daily living, bathing, dressing and grooming.
- Responds to residents and family inquiries and calls for assistance.
- Maintains supplies to include ordering, stocking and monitoring for expiration.
- Documents services performed and supplies provided for billing purposes.
- Delegates assignments to Certified Nursing Assistants as necessary.
- Assist in the monitoring of the Restorative Program and report pertinent information to the appropriate staff.
- During emergencies or natural disasters, you may be required to report for duty as assigned by the Administrator or his / her designee, for the duration of the disaster.
- Performs restorative and rehabilitative nursing for residents by communicating the instructing in activities of daily living or other self - help techniques as outlined in the resident care plan.
- Makes rounds and tours on a regular basis assessing resident needs and appearance of facility, verifying resident needs are being met.
- Responds to emergency calls as needed within the community.
- Communicate with residents, family, friends, staff and all medical personnel (for example, doctor, pharmacy, lab hospice, EMS, etc.) on resident needs and / or concerns in a professional manner.
- Relates an accurate and thorough report to the oncoming shift to ensure the continuity of maximum resident's care.
- Responsible for timely documentation of written and verbal reports on any / all unusual or abnormal observations, emergency care, resident needs, family concerns, change in physician's orders, narcotic records, follow - up care, resident care plan revisions, any / all changes related to ensuring that quality resident care is provided, and staff incident reports are handled appropriately.
- Report any suspicious behaviors such as abuse, neglect, theft or any unlawful act against a resident to your supervisor immediately (as defined by the Elder Justice Act).
- Perform routine inspection of equipment (for example, walker, nebulizer, furniture, etc) to determine maintenance needs and cleanliness.
- Assist in the development of programs to educate and enhance employee skills directed at improving the quality of life of the residents.
