POSITION SUMMARY :
Under the direct supervision of the Case Manager Supervisor, in the Family Rehousing and Stabilization Program ( FRSP ), the Case Manager coordinate all Provider type (including Secondary Service Providers, District Agency Providers, and Community Providers) services and ensure that families are making meaningful progress toward increasing earnings and achieving housing stability.
The incumbent is responsible for conducting outreach to referred families and administering the 2Gen approach, engaging families in Provider activities, re-engaging those families who started to participate and subsequently stopped, introducing families to Provider processes, reviewing DHS-administered assessments, developing, and amending housing stabilization plans, and helping families overcome barriers to program engagement and participation.
The Case Manager is responsible for developing trusted working relationships with assigned families and will monitor families’ engagement and progress for as long as the families are in the program. The incumbent will coordinate multiple activities outlined in the housing stabilization plan.
The Case Manager is ultimately responsible for the engagement of families and should work with TEP Providers to identify appropriate education and occupational training programs as well as employment opportunities for families. The incumbent directly provides case management reports, statistics, and plans for review by necessary stakeholders and provides accurate and timely processing for his / her assigned cases.
The Case Manager correctly assesses clients’ needs, preferences, and abilities and uses the information to develop tailored care plans. The incumbent links clients to additional East River or community resources through internal / external referrals as needed.

Programmatic : (60%)
Works closely with families to ensure there is a plan in place to work towards increasing earnings and housing stability.
Conducts outreach efforts to engage families in the community and educate them about FRSP and other resources offered by East River.
Makes necessary referrals to other community resources as dictated by a family’s situation.
Connects with families who once were participating in the FRSP or other programs to share resources.
Continuously works with families and the Case Manager Supervisor to develop and administer achievable stabilization plans.
Identifies potential barriers for families and develops creative ways to overcome those barriers, leading to FRSP participation, or connection to other resources.
Coordinates, leads, and coaches families through their specific plans and serves as a reliable resource to overcome obstacles.
Serves as a motivator for families to ensure continued participation and improvement.
Connects with the Housing Coordinator and Employment Specialist to provide a holistic approach to case management plans and the families’ goals.
Participates in training to learn more about effective case management to better help FRSP participants.
Regularly maintains relationships with other community agencies, educational institutions, businesses, and service providers to advocate for FRSP families.
Regularly integrates / links families to informal and formal resources within their community or neighborhood to create natural support systems.
Regularly participate in internal and external meetings in the spirit of mutual support of families and advocate for needed resources and services and ensure families are provided with all possible resources available.
Serves as a genuine, sound, and reliable resource to families within the scope of the FRSP and East River guidelines.
Case Management Compliance : (25%)
Completes end-of-month reports in an accurate and timely manner and connects with the Case Manager Supervisor, FRSP to obtain feedback, support, and guidance as needed.
Utilizes case management best practices, East River policies and procedures, funder requirements, and quality assurance guidelines to execute proper case management.
Participates in periodic case management training as required.
Works closely with clients and the supervisor to resolve complex case management compliance concerns.
Files and reports physical and electronic case information in accordance with the compliance requirements.
Enters information in an East River-approved electronic database as required.
Connects with clients as needed to obtain missing or outdated information to ensure reporting is accurate at all times.
Frequently reviews caseload documents, reports, statistics, notes, and other information for accuracy and compliance.
Administrative : (10%)
Compiles case management statistics for monthly reporting.
Reviews unusual incident reports and recommends corrective action.
Regularly reviews documentation (case records and database content) to ensure compliance with contract and East River policies.
Reviews reports, plans, and management reports ensuring all information is accurate, up to date, and contains necessary information.
Ensures all FRSP program administrative requirements are met.
Assists the Case Manager Supervisor, FRSP with various reports as requested.
Maintains all paperwork regarding the program and families and ensures the necessary electronic and / or physical filing as required.
Manages data on spreadsheets and Word documents as required.
Answers phone calls in a courteous, professional, and helpful manner.
Generates cyclic and ad hoc reports as needed and as requested by internal and external stakeholders.
Responsible for clerical duties, including answering phones and preparing documents.
Contributes to staff meetings and briefings as required.
Submits East River forms and documentation in a timely manner.
Miscellaneous : (5%)
Ensures that the mission and vision of the agency are carried out as it relates to the FRSP.
Maintains strict confidentiality in all matters, including but not limited to, clients, personnel, budgets, contracts, Board of Directors, and all other non-public business information.
Other duties as assigned.

Education / Work Experience :
