Job Summary
East Tennessee State University, College of Nursing, is seeking a Care Coordinator (RN) at the Johnson City Community Health Center (JCCHC), a state-of-the-art facility on par with any of its kind in the United States. Funded by a generous grant from the U.S. Department of Health Resources and Services Administration, it was established through the vision of leaders at ETSU, the College of Nursing and the College of Clinical and Rehabilitative Health Sciences, all of whom are dedicated to providing access to basic health care services for all in our community and region.
Eligible for Sign-On Bonus - $500 after 30 days. $1000 for continued employment after 6-months within the CON OOP department as an initial sign-on bonus. A sign-on bonus is not eligible when transferring or being promoted to a new position within the CON OOP.
The Care Coordinator partners with chronically ill or high-risk patients, their families, healthcare providers, and community resources to ensure timely access to care, increase preventative services, and reduce hospital readmissions and emergency visits. By developing personalized care plans and offering culturally appropriate education, the Care Coordinator empowers patients to manage their health more effectively and improves adherence to treatment.
This role involves daily one-on-one patient meetings, coordinating follow-ups with healthcare teams, and connecting patients to community resources. Success in this role leads to better patient outcomes, increased satisfaction, and reduced healthcare costs. The FLSA status (exempt or non-exempt) will depend on the incumbent's entry salary.

Knowledge, Skills and Abilities

Physical Demands

While performing the duties of this job, the employee is occasionally required to position themself and balance. The employee must occasionally move (including lift, carry, push, and pull) up to 100 pounds or more. The employee may be required to move and / or stay in a stationary position in various settings for up to 12 hours at a time in addition to preparing and closing a clinical shift. Stress levels may be high due to frequently dealing with numerous students, patients, family members, and other healthcare providers. This position demands concentration and the ability to work in emotionally charged situations.
