Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) whose services include providing personalized, affordable, high-quality medical, dental, vision and behavioral health care through a community-based network within the East San Gabriel Valley and Pomona Communities. Our mission is to provide access to excellent health care while engaging and empowering our patients, employees, and partners to improve their well-being and the health of our communities.Major Positions Responsibilities and Functions : Provide CPSP orientations via one (1) hour face-to-face session.Provide Prenatal Combined Assessment / Reassessment Tool and Combined postpartum assessment per CPSP guidelines - a minimum of three (3) one-hour face-to-face assessments per client.Based on assessment tool responses, provide one-to-one nutrition, psychosocial counseling, and health education to clients.Develop individual care plans and conduct appropriate follow-ups for clients, in person and via phone calls.Assist in the development / revision of assessment tool protocol.Consult with psychosocial and nutrition consultants as appropriate.Report / document client progress needs in medical charts.Ensure / assist clients with practical arrangements for meeting medical, diagnostic, specialty consults, nutrition, psychosocial and other recommendations.Assure that perinatology and obstetrical diagnostic reports are received and shared with the provider.Per provider order, contact the patient regarding follow-up visits or other medical issues.Assist patients with hospital pre-registration.Assist patients with form completion (disability, FMLA, etc.)Follow-up on Medi-Cal eligibility issues.Track patient data, including required UDS reporting, and complete reports as required.Attending Comprehensive Prenatal Health Worker training.Participates in training provided by the CPSP program to further educate and keep current with industry changes, and clinical requirements.Collects and reports statistical information.Attends department staff meetings including case conference meetings.Assists with CPSP audits.Other duties as assigned.Position Requirements and Qualifications : Ability to work with adults and youth in a variety of settings.Basic knowledge of reproduction, contraception, and related topics.CHW Certification and counseling skills.Bilingual in English / Spanish preferred.One year of prenatal care experience.Excellent verbal communication skills.Written / oral fluency in Spanish.Strong written and organizational skills.DOE : $21.61 - $26.24East Valley offers a competitive salary, and excellent benefits including medical, dental, vision, and defined contribution retirement plan.
