Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) who's services include providing personalized, affordable, high-quality medical, dental, vision and behavioral health care through a community-based network within the East San Gabriel Valley and Pomona Communities. Our staff practices patient-centered care by serving each patient with a personalized care plan that meets their individual needs. Our patients have access to support services that include, nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations. East Valley serves the health care needs of uninsured and underserved individuals and families throughout our 8 health center locations.

Essential Position Functions and Duties :
- Responsible for patient's vitals including blood pressure, weight, height, temperature, pulse and history of present illness.
- Documents chief complaints as stated by patient.
- Provides immunizations, draws blood, perform hematocrit and other basic test when requested by clinicians.
- Calls in medication to pharmacy and administers medication as ordered by medical providers.
- Receives related telephone calls and triages appropriately, including managing patient messages per protocol or medical provider instructions.
- Reviews all forms to insure that charts are completed after each patient visit.
- Responsible for providing final instruction and patient education as directed by medical provider.
Qualifications :
DOE : $22.00 - $25.98
