Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) whose services include providing personalized, affordable, high-quality medical, dental, vision, and behavioral health care through a community-based network within the East San Gabriel Valley and Pomona Communities. Our staff practices patient-centered care by serving each patient with a personalized care plan that meets their needs. Our patients have access to support services that include, nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations. East Valley serves the healthcare needs of uninsured and underserved individuals and families throughout our 8 health center locations.

Position Responsibilities and Functions :
- Enter referral authorization requests into both the IPA and County electronic systems.
- Coordinate and follow up on referrals for specialties or diagnostics.
- Communicate with providers and patients.
- Demonstrate ability to prioritize and handle urgent referrals.
- Maintain electronic logs of all referrals in the i2i Population Health system
- Enter pertinent medical information into the order management section in NextGen.
- Enter referral authorization requests into both the IPA and County electronic system.
- Obtain consultation reports.
- Attend meetings offsite / onsite / zoom.
- Work cooperatively with all members of the healthcare team.
- Work with the health care team to ensure prompt processing of referrals.
- Mail out authorization letters.
- Follow up with patients as needed or as requested.
- Answer incoming calls, and respond to voice messages.
- Attending assigned training either offsite or onsite.
- Respond promptly and appropriately to patient and / or provider requests.
- Demonstrate dignity with kindness and compassion to all patients, visitors, and staff.
- Scan all referral requests, authorizations, and consult notes into the patient's electronic medical record.
- Maintain confidentiality of sensitive information
- Other responsibilities as assigned.
Position Requirements and Qualifications :
Excellent Communication skills both verbal and written.
DOE $22.00 - $25.98
