About Us: Under the umbrella of the Eastern Nebraska Human Services Agency (ENHSA), the Eastern Nebraska Office On Aging (ENOA) works to ensure older adults within the region have access to services that allow them to remain active and independent. These services operate to fill gaps in existing services within a 5-county radius and include nutrition programs, care management services, volunteer opportunities, and community services.

For the success of the position, we require the following:

  • Bachelor’s degree in human services, education, or health/medical field OR licensed RN with minimum 2 years professional experience in long-term care
  • Previous work experience and understanding of psycho-physiological traits of aging preferred
  • Must have a valid Nebraska or Iowa Driver’s License (or ability to obtain within 30 days)
  • Fairly clean driving record with State required car insurance
  • Successfully complete drug screen
  • Must not appear on APS/CPS Registry for abuse/neglect, or have been dismissed from prior employment due to suspected abuse/neglect
  • Professionalism & Accountability

Job Summary: This position determines and monitors the appropriate long-term care needs of financially eligible disabled adults and/or older adults who are requesting or receiving assistance through home and community-based services. By using a participant-centered approach, you will identify the clients’ strengths and needs and align that with formal and informal supports and services.

Core Responsibilities:

  • Assessment – identify strengths and limitations of clients
  • Planning – based on the comprehensive assessment, create a collaborative, written document outlining services and supports
  • Implementation – identify formal providers and informal supports focusing on cost effectiveness and participant preference
  • Coordination – develop and maintain rapport with participant and family/caregivers acting as “team leader” initiating communication with other disciplines as needed
  • Monitoring – follow-up to ensure services were provided and participant satisfaction
  • Evaluation – determine if goals and objectives in the plan of services and supports produce beneficial results to the client and agency
  • Advocacy – provide client with information and training that promotes self-sufficiency

Knowledge, Skills, Abilities:

  • Basic knowledge of applicable state and federal laws, policies, and regulations as they relate to Nebraska Medicaid Program
  • Knowledge of medical and psychiatric diagnoses, prognoses, needs, and expected outcome goals.
  • Ability to work independently and collaboratively
  • Overall computer proficiency and ability to learn new systems
  • Strong ability to prioritize tasks and use critical thinking
  • Possess good organizational skills
  • Strong standard of integrity and professional ethics
  • Bilingual English/Spanish Preferred
