Summary
Provide direct patient care under the direction and supervision of a registered nurse in a clinical setting.
Qualification Requirements
To perform this job successfully, an individual must be able to participate in the assessment of patients; use critical thinking skills in response to events, observations, experiences, verbal and written communication, problem solving and decision making.
Minimum Qualifications
- Current license to practice as an LPN in the state of North Carolina.
- Current BLS or ACLS certifications
- Preferred: 2 years of Nephrology experience
- Passage of a criminal background check, OIG exclusion and drug screenings
- Valid driver’s license and reliable transportation
Essential Functions
- Gathers necessary information to determine the needs prior to the doctor’s exam. 25%
- Perform laboratory work with scope of practice. 25%
- Check HGB of patient via fingerstick after training and certification by the Physician(s), Nurse Manager, Nurse Practitioner(s), Physician Assistant(s), or other Qualified Personnel as required per protocol and document in the patient’s electronic medical record (EMR). Participates in Chronic Care Management 10%
- Gives instructions to patients as instructed/ordered by doctor. Makes specialty referrals as ordered by doctor and documents medical information in the electronic medical record. 25%
- Maintain accurate medical records by documenting patient communication, changes in intervention per medical provider, referral calls, pharmacy calls, and triage calls in the EMR. 15%
