The Wraparound Care Coordinator plays a vital role in supporting youth and families by working collaboratively with community partners and the MCCFL Clinical Services Team. This position focuses on enhancing each client’s ability to actively engage in services while ensuring adherence to the Wraparound process and principles.
Given the high prevalence of trauma among individuals receiving mental health services, the Care Coordinator must have a strong understanding of how trauma impacts health, relationships, coping strategies, and other aspects of daily life. The goal is to empower youth and families by facilitating their individualized goals through a strengths-based, family-centered approach.
Key responsibilities include:
- Initiating engagement with parents and youth.
- Supporting the development of a comprehensive team, incorporating both natural and professional supports.
- Facilitating regular team meetings.
- Developing and implementing individualized family care plans that reflect the youth and family’s needs and preferences.
This role involves working with youth and families who are engaged with multiple systems, such as juvenile justice, child welfare, substance abuse treatment, and special education. A deep understanding of public service systems and the ability to build strong community connections are essential for ensuring client stabilization. Clients may present with diverse mental health challenges, including trauma history, emotional or behavioral dysregulation, suicidality, attachment difficulties, and relationship challenges.
Qualifications & Requirements:
A combination of education, certification, and experience demonstrating the ability to fulfill the role’s responsibilities, including:
- Certification as a Qualified Mental Health Associate (QMHA) by the Mental Health and Addiction Certification Board of Oregon (MHACBO) or the ability to register as a QMHA-R within 14 days of hire.
- Completion of Oregon Wraparound Foundations Training within six months of hire.
- Oregon CANS 0.5 2.0 and Oregon CANS 6-20 2.0 certification within 30 days of hire.
- A minimum of two (2) years of relevant experience (preferred).
- Bilingual fluency in Spanish/English (preferred).
- Ability to pass all criminal background checks in compliance with ORS 181.536-181.537 and OAR 407-007-0200 to 407-007-0370.
- A valid Oregon or Washington driver’s license, a clean driving record, and the ability to maintain insurance for client services and travel between locations.
