Job Overview
Eastern Oregon Center for Independent Living (EOCIL) is seeking a dedicated, compassionate, and detail-oriented Care Coordinator (CC)to join our team. The CC will support individuals living with HIV/AIDS in accessing and adhering to medical care and treatment. This role is essential in promoting overall health and wellbeing while conducting outreach to underserved communities across our service areas.
Responsibilities
- Provide care coordination services to individuals living with HIV/AIDS.
- Assist clients in navigating healthcare systems and social services.
- Assist with the preparation of client records and documentation.
- Provide support by handling phone calls, emails, and inquiries regarding client care.
- Support clients in developing their care plans.
- Maintain accurate records of client interactions and follow up on care plans as necessary.
- Ensure compliance with Oregon HIV Care Services Regional Model requirements and maintain confidentiality of client information.
- Delivery of Trauma-informed and harm-reduction services, including mental health referrals and access to harm-reduction services.
- Coordinate with multidisciplinary care team, including medical case managers, program director, to optimize care outcomes.
- Participate in quality management meetings, ensuring continues improvement and compliance with best practices in HIV case management.
Qualifications
- A degree in human services field, or one year full-time equivalent experience in a related role.
- Previous experience working in an office setting is preferred.
- A valid driver's license, for offsite outreach, meetings and events.
- Must be able to pass a criminal history background check.
- Bilingual (Spanish/English) candidates are strongly preferred.
- Excellent organizational skills with the ability to manage multiple tasks simultaneously.
- Understanding of culturally relevant interventions for high-risk communities, including Spanish-speaking populations, LGBTQIA 2S individuals, people who inject drugs, and those living HIV/AIDS.
- Strong interpersonal skills with a focus on customer service and client satisfaction.
- Familiarity with local and statewide healthcare and social service providers.
- Ability to work collaboratively within a team environment while also being self-motivated.
- High attention to detail and accuracy in all aspects of work.
