What are the responsibilities and job description for the Mental Health Case Manager position at Eastern Panhandle Mental Health Center?
The Case Manager is responsible for providing high quality care in the program in which they work. The Case Manager is responsible for utilizing good skills in assessment, treatment planning, case management, group planning and facilitation, and the documentation of services. The Case Manager is expected to function well as a member of the Clinical Team. Responsible for providing targeted case management services and assessments, service planning and evaluation, linking, advocacy, and crisis intervention to assigned case load. Responsible for providing individual, family, marital, and group therapies.
