What are the responsibilities and job description for the Pre-ETS Transition Consultant position at Easterseals Arc of Northeast Indiana?
Summary: Easterseals Arc of Northeast Indiana seeks a Coordinator to join its Pre-Employment Transitions Services program. As a Pre-ETS Coordinator you will provide services to students with disabilities to improve their outcomes after high school and assist them with a positive transition from high school to adult life. Such services may include but are not limited to developing plans for work-life skills training and experiences based on the student's Individualized Education Plan (IEP) and personal goals.
Maintain compliance with all regulatory bodies.
Ensures that services adhere to regulations.
Maintains detailed client case notes, compiles, and submits reports.
Ensures we are meeting the goal of Vocational Rehabilitation's Pre-ETS contract
Accurately tracks, monitors, and documents all approved billing
Ensures accurate and proper documentation of services; ensures that services adhere to regulations
Adheres to Easterseals Arc Human Resources and EEO/AA policies and performs other duties as assigned
Supervises assigned personnel, including interviewing, hiring, training, disciplining, and terminating, reviewing and approving work schedules. Shall Perform duties of absent personnel as needed.
Ensure services provided are individualized
Partners with schools to develop and carry out an individual person-centered plans in alignment with Pre-ETS services
Provides resources to families and school personnel regarding pre-employment services
Performs duties and develops work training opportunities for participants
Accurately documents all services provided and ensures they are individualized unless stated differently
Respect confidentiality and deliver services in a culturally competent fashion that is also respectful
Facilitates learning within a classroom and community settings
Attends meeting regarding Pre-ETS or for the students being served within Pre-ETS
Preserve, support, and stabilize families, and promote the well-being of children, adults and families
Applies for and refers to community employment resources and or post-secondary resources
Assist with the supervision of high school students
Help with maintaining the work/training environment to ensure safety at all times
Strives to always put the needs of the student first
Reports and notifies the supervisor of any immediate issues or problems that may be seen.
Ensures all work site locations are safe and optimal for the students
Maintain a positive and supportive environment with all family members
Uphold agency values and follow agency policies and procedures
Maintain confidentiality of records/information according to HIPAA, State, Federal Laws and guidelines
Maximize and responsibly utilize agency and community resources
Serves as liaison between Easterseals Arc and the designated location
Ability work remotely and exercise good judgement and common sense
Qualifications:
At least 18 years of age required.
High School diploma or GED preferred.
Ability to pass criminal/registry background checks.
Must have a current driver's license, proof of insurance, safe driving history, and car to drive.
Experience working with young adults, aged 14-22; prefer 1 year.
Experience working with individuals with disabilities; prefer 1 year.
Experience supervising/leading a small group of staff; prefer 1 year
Working Conditions: Pre-ETS Coordinators work in a changing environment that adjusts to the needs of the individuals in program services. Coordinators need to be flexible and be able to adjust to changing needs, activities, and locations. Coordinators must have an insured, safe vehicle in good working order to utilize for possible transportation required for staff and program participants to and from service locations.
PHYSICAL REQUIREMENT- Depending on the needs of the individuals, the work can be physically demanding including transferring or supporting people with mobility needs, lifting or moving items up to 75 lbs, walking or standing for extended periods of time or providing support through CPR/First Aid, behavioral support, and other interventions. Staff may work with violent/physically aggressive participants and responds to emergency situations involving potential physical harm to self and others
