POSITION TITLE : Early Intervention Service Coordinator
POSITION LOCATION : Child and Family Connections #14 – Henry & Stark Counties, Illinois
POSITION TYPE : Full time (40 hours / week)
POSITION STATUS : Work from home with State of Illinois residency requirements : candidates must live in either Henry County, Illinois or Stark County, Illinois to be considered for this position
Qualifications
- Candidates are required by the State of Illinois to have a Bachelors degree or higher from an accredited college or university in Education, Social Work, Human Services, Behavioral Science, or a health-related field
- Candidates are required by the State of Illinois to live within either Henry County, Illinois or Stark County, Illinois
- Official Transcripts from college / university
- Eligible for Early Intervention Service Coordination Credential
- Proven successful experience working with children and families preferred
Child and Family Connections of Central Illinois is the regional intake agency for children birth to three and their families to enter the Illinois Early Intervention System. Early Intervention (EI) credentialed Service Coordinators work with families to develop an Individualized Family Service Plan and select service providers to meet the individualized needs of their child. Child and Family Connections can also assist with Social Emotional Consultative Services and free developmental screenings through the Local Interagency Councils.
Position Objective
The Early Intervention Service Coordinator provides in-home or center-based consultation services and are responsible for utilizing a variety of formalized assessment tools for the child and / or other family members. The Service Coordinator coordinates with other disciplines and agencies to ensure a comprehensive assessment; reviews family resources, relationships and attitudes, and makes independent clinical judgements regarding appropriate services to be provided to client and family.
