Position Opening – Case Manager & Outreach Associate

Easterseals Delaware & Maryland’s Eastern Shore .is seeking a Case Manager and Outreach Associate to join the Community Outreach Program team in our Milford location. Key job responsibilities:
- Provide case management services to Caregivers and other consumers contacting the Community Outreach Program
- Serve as a resource for information and referral
- Complete funder documentation per required timelines
- Offer education to caregivers and their families
- Participate in outreach activities to assist with program expansion efforts
Qualifications:
- Bachelor’s degree or advanced degree in a related field
- Demonstrated ability to work with a wide range of disabilities & age groups
- Knowledge of resources in Delaware, particularly Sussex County
- Willingness and ability to drive throughout the state

This full-time, 40-hour per week position is based out of our office at the Milford Wellness Village. Hours are typically Monday-Friday, with the occasional evening or weekend for outreach events. Mileage reimbursement is offered for work-related travel. The compensation for this role is based on experience.
