Essential Job Functions
- Empower Individuals: Provide support to help individuals achieve independence and live their version of a 'good life.'
- Implement Plans: Collaborate with the Case Coordinator to provide input on and implement Individual Plans for individuals.
- Promote Health & Safety: Follow organizational guidelines to ensure individuals' well-being.
- Monitor & Report: Address concerns promptly, escalating issues to the Case Coordinator or supervisor as needed.
- Provide Personal Care: Assist with daily living activities (toileting, dressing, feeding, etc.) while maintaining individuals' privacy and dignity.
- Complete Documentation: Ensure all individual-related data and documentation are accurate and submitted by deadlines via web-based platforms.
- Administrative Tasks: Submit timesheets, leave requests, and other required documents on time.
- Attend Meetings & Trainings: Actively participate in departmental meetings and required trainings.
- Maintain Cleanliness: Assist in maintaining a safe, hygienic, and clean environment.
- Respond to Emergencies: Take appropriate action during emergencies, following established procedures.
- Handle Multiple Tasks: Balance various responsibilities, meet deadlines, and ensure attention to detail.
- Safety Reporting: Report safety concerns promptly to the Program Manager or supervisor.
- Other Duties: Complete additional responsibilities as assigned.
Qualifications
- Minimum Education: Must be at least 18 years of age with a High School Diploma or equivalent.
- Knowledge, Skills, & Abilities: Proficiency in communication, HIPAA compliance, and technology usage.
- Physical Requirements: Capability to lift up to 50 lbs and assist with physical transfers using equipment like a Hoyer lift.

Physical Requirements:
- Frequent bending, reaching, squatting, kneeling, twisting when working with participants.
- Responsive to audio and visual cues from participants and their families, other staff members, and the environment
- Work calmly during behavioral and health-related incidents.
- Use logic and reasoning for anticipating future needs, goals, and events.
- Stand or sit for extended periods of time, stand for up to 6-8 hours a day.
- Occasional lifting, carrying, pushing, and loading of materials up to and including 50lbs.
- Provide assistance during transfers when applicable. Some participants may require the use of a Hoyer lift for transfers.
- Frequent driving (to and from office, the community, and participants’ homes) when applicable.
- Work at participants’ homes, community settings, and ESH facility, depending on assignment.
Conditions of Employment
- Ability to obtain and maintain criminal record clearance through the Department of Justice (DOJ). The People & Performance Department must analyze DOJ/FBI live scan reports in accordance with applicable Federal, State, and Local laws, as well as fitness for the position.
- Able to travel to multiple work sites and client homes. Reliable transportation needed.
- Must have valid Hawaii Driver’s License/insurance and maintain a clean driving record.
- Must pass health screenings, obtain vaccinations and clear TB testing based on company policies.
- Ability to obtain and maintain clearance through the Office of Inspector General.
- Ability to obtain and maintain CPR certification.
- Ability to obtain and maintain Safety Care certification. [For credentialed roles]
- Ability to obtain and maintain a CAQH credential. [For credentialed roles]
- Ability to obtain and maintain RBT certification. [For BIs, PAs]
- Ability to obtain and maintain NPI number.
- Must attend any required training.

The listed salary range for this position is an expected range. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
