Essential Job functions:
- Analyzes clinical histories for diagnostic purposes, and develops individualized treatment plans for individuals.
- Conducts clinical interviews and provides individual, group, conjoint or family psychotherapy to carry out the treatment plans to attain individual goals.
- Records the treatment plan, progress and other significant information regarding the individuals for agency case records.
- Participates as an active team member on an interdisciplinary team to triage clinical interventions and concerns.
- May administer, score and interpret psychometric and/or projective tests to aid in developing diagnoses and providing treatment services.
- May perform case management functions and act as a liaison with school and/or vocational personnel and other community agencies to refer clients, formulate and execute treatment plans, and provide educational and consultative services.
- Performs assessments for diagnostic purposes, interviews individuals and others related to the case, and reviews applicable case history to determine needs.
- Develops treatment plans and prioritize the individual needs of those receiving services including recommendations for appropriate community based services.
- Provides therapeutic counseling on a limited, short term basis and offers crisis intervention as needed. Evaluates the progress of individuals on an on going basis.
- Responsible for authorizing medically necessary services for all individuals.
- Demonstrates knowledge of principles of trauma informed care.
- Willingness to support and assist individuals with trauma related issues.
- Completes and maintains all paperwork in accordance with Medicaid and Department of Community Health guidelines, as well as Easterseals policies and procedures.
- Performs crisis intervention as needed. (Phone after hours coverage required 1 week per year )
- Attends monthly team meetings to review problems and plan solutions.
- For children/families served, ensures that all have CAFAS/PECFAS completed quarterly.
Minimum Qualifications:
- Possess a Master’s degree from an accredited college or university with a major in Social Work; AND Have an LLMSW or LMSW from the State of Michigan Department of Consumer and Industry Services. LLPC/ LPC also accepted.
- Must meet qualifications of a Child Mental Health Professional (CMHP) and/or Mental Health Professional (MHP) per the Michigan PIHP/CMHSP Provider Qualifications per Medicaid Services
- Possess a valid motor vehicle operator or chauffeur's license, and auto insurance.
- Must possess knowledge and have an understanding of co-occurring disorders (mental health and substance abuse).
