We are seeking a highly skilled Clinical Care Manager to join our team. In this role, you will be responsible for assessing and coordinating care and support services for adults with mental illness and children with emotional disturbance and their family who are experiencing crisis. You will work closely with our multidisciplinary team to ensure that individuals receive the highest quality care and support.

To be successful in this role, you must have an LMSW, LLMSW, LLPC, or LPC from the State of Michigan Department of Consumer and Industry Services. You must also meet qualifications of a Child Mental Health Professional (CMHP) and/or Mental Health Professional (MHP) per the Michigan PIHP/CMHSP Provider Qualifications per Medicaid Services.

Responsibilities
- Assess and develop treatment plans for individuals with mental illness and emotional disturbances.
- Coordinate care and support services, including behavioral health and other needed services.
- Collaborate with community resources and providers to ensure seamless service delivery.
- Maintain accurate and up-to-date electronic records, including required documentation of services provided and progress notes.
