Job Duties
- Assesses and coordinates care and support services for adults with mental illness and children with emotional disturbance and their family who are experiencing crisis.
- Serves as the outpatient clinician for the crisis stabilization team.
- Conducts clinical assessments, develops diagnoses, and participates in developing person / family centered treatment plans with individuals served and their families. Monitors progress and coordinates appropriate behavioral health and other needed services including, but not limited to, community coordination, outreach, and referrals.
- Responsible for maintaining an electronic record which includes required documentation of services provided, progress notes, treatment plans, periodic reviews and rights documents for persons served.
- Participates in individual supervision and team meetings to review progress, identify barriers and develop strategies for better assisting individuals and families in reaching their goals.
- Responsible for full compliance of individuals on caseload having CAFAS / PECFAS / LOCUS / DLA completed quarterly or as identified in clinical record.
- Provides crisis intervention and stabilization to those referred by other clinicians.
- Ensures that paperwork is maintained in accordance with funder requirements as well as Easterseals MORC policies and procedures.
- Responsible for assessing, reviewing all requests for services and approving / authorizing medically necessary services for all individuals served. Only implements therapeutic modalities approved by Easterseals MORC.
- Demonstrates knowledge of principles of trauma informed care and co-occurring disorders.
- Willingness to support and assist individuals served with trauma related issues.
