POSTION TITLE: Veterans Housing Case Manager

DEPARTMENT: Supportive Services for Veteran Families (SSVF)

LOCATION: Portland, OR

MISSION: Assist Veterans and their families with housing stabilization through person center focused case management.

ESSENTIAL FUNCTIONS:

1. Determine program eligibility and process intake/enrollment. Provide assessment and goal planning. Maintain compliance with Easterseals Oregon’s policies and procedures, and the SSVF program rules and regulations.

2. The Case Manager will assist clients with develop of Individual Action Plans and help establish connections to resources that will assist the participants reach their goals. Case Manager will provide financial assistance, supports to assist participants in stabilizing their housing, and will provide regular assessments to determine eligibility, level of financial assistance, and length of needed support services.

3. The Case Manager will connect participants with community resources and supportive services for which they may be eligible, including health care services, daily living services, transportation services, childcare services, legal services, personal financial planning services, fiduciary and payee services, and others as appropriate to the needs of each household. The Case Manager will work closely with collaborating community programs to enhance supports for the participant.

4. Maintain accurate, up-to-date records and case files on the veteran and report regularly to the Program Manager. Data entry and collection for reporting and record keeping.

5. Counsel participants about employment and facilitate appropriate linkages between the veteran and community resource agencies. Advocate on behalf of veteran with these agencies.

6. Assist participants with resolving possible barriers to housing, transportation issues, childcare, employment and/or medical needs.

7. Network with and maintain positive relationships with Oregon Department of Veterans Affairs, VA Medical Center, Veterans Community Resource Referral Center, Veterans Services Officers, HUD VASH providers and Community partners.

8. Stays informed of community, agency, and program information through inter-office communications, attendance at staff meetings, participation in professional development activities and other community events

9. Complete other assignments as requested by the Program Director and Program Manager.

Minimum Education, Certification, Experience:

Ø Bachelors’ degree in social services or related field or 2 years’ case management experience

Ø Demonstrated interest in working with Veterans and the ability to be compassionate and understanding

Ø Demonstrated ability to work with others and communicate effectively

Ø Ability to work with individuals of varying incomes, skill levels, experiences and cultures

Ø A professional appearance

Ø Strong Information technology skills

Ø Ability to manage multiple tasks simultaneously

Other Requirements:

MVR background check

Valid driver’s license

Current vehicle and liability insurance

Job Type: Full-time

Pay: From $23.50 per hour

Expected hours: 40 per week
